SCREENING & TREATING REPORT

After participating in VA HEPC training, practices should complete this form at the end of each month to report the patients screened and treated. If your practice is not treating, or you have not screened or treated during a month, please complete the form and fill in zeros.

Sharing this information helps Virginia HEPC accurately track the impact of its training program. Please take the time to report consistently!

RELINK UPDATE

VA HEPC received a grant from CDAF ReLink to link diagnosed but untreated HCV patients to care. As of today 5,122 patients with HCV diagnosis have been identified and 2,794 have been treated or are being linked to treatment. We continue to work on developing a sustainable process to ensure all diagnosed patients are linked to treatment in the future.

COMMUNITY OF PRACTICE

Join us virtually for Virginia HEPC Community of Practice (COP) on the third Wednesday of each month from 4pm to 5pm. On November 20, we’ll be discussing infectious disease and IV drug use with Rebecca Dillingham, MD/MPH Medical Director, Virginia HEPC Infectious Disease Specialist
