RELINK PROJECT

VA HEPC received a grant from CDAF ReLink to link diagnosed but untreated HCV patients to care. This grant allows for In-Reach into the Sentara system. Patients with HCV diagnosis have been identified and we are developing a system to link those patients to treatment. This grant allows us to implement a sustainable process to ensure all diagnosed patients are linked to treatment in the future.
