Screening & Treatment Reporting
Help us track our impact!
Every month, Virginia HEPC asks providers in its network to report on their site’s screening and treatment activity. The information can be entered into a a HIPAA-secure REDCap form directly, or by using the secure ‘Upload file’ link on the REDCap form to upload your file.
Please complete the report each month. If your site has not screened or treated patients (because you haven’t started your program, or didn’t have any referrals) please enter zeros and submit the form. It helps us to know when you haven’t screened or treated.

HEPC Treatment Reporting FAQs
What information do I need to submit?
We ask for the number of patients for the following categories: screened, first appointment, prescribed treatment for HCV, started treatment for HCV, completed treatment for HCV, completed labs 12-weeks after treatment, undetectable viral load 12-weeks after treatment (cured).
Why do you collect this information?
By providing information on the number of people screened, in treatment, and cured, VA HEPC can track the impact our network is maing on eliminating HCV in Virginia.
What if people from our site attended training, but we haven’t started treating independently?
Once your site has trained with us, you are considered part of our network. It is fine if it takes some time to start treating. Each month after training, however, we ask you to complete the Treatment Report. If the ‘Number of patients’ answer is zero for any question, simply mark those items as zero. It is helpful for us to know when your site hasn’t screened or treated.
We already track this info using an Excel worksheet, will you accept that?
Yes! When you open the form, look for the ‘Upload file’ link to upload your own tracking file. Our team can get the numbers from your file. Report in the way that is easiest for you.
Do you have an Excel worksheet to use as a template for the data you want sites to report on?

Is sharing this information HIPAA-compliant?
Yes! The REDCap form and ‘Upload file’ link are HIPAA compliant.
What if I miss a month of Treatment Reporting?
We understand that things can get busy. You put in the start and end dates for the information you submit. Do your best to report as soon as you can. Let us know by email if you can’t submit your report.
