Information for Clinicians

Those being released from incarceration face many barriers while re-entering society. HEPC helps connect people who were diagnosed with hepatitis C while incarcerated, but not treated, to treatment on release. Through a collaboration with the Virginia Department of Corrections (VADOC), HEPC receives expected contact information for those living with hepatitis C that will soon be released. HEPC provides counseling to releasing inmates through telemedicine, which includes hepatitis C education, making them aware of the HEPC program and providing our contact information.

HEPC reaches out soon after their expected release date to see if they are ready to pursue treatment. If so, we assist in navigating the complex medical systems to lower barriers to referral sites and schedule their first appointment. If they are not ready for treatment, we make sure they have our contact information and ask if we can contact them again later to check in on their readiness.

In addition to scheduling the first appointment for those wishing to pursue treatment with a referral location, we follow up with patients and sites to ensure patients can access treatment. Too often, these patients get lost between the steps of accessing care, so we strive to ensure this does not occur.
