Help us track our impact!

Every month, Virginia HEPC asks providers in its network to report on their site’s screening and treatment activity. The information can be entered into a a HIPAA-secure REDCap form directly, or by using the secure ‘Upload file’ link on the REDCap form to upload your file. If you are tracking patients using our Patient Tracking Worksheet, you can upload that file using the REDCap form.

Please complete the report each month. If your site has not screened or treated patients (because you haven’t started your program, or didn’t have any referrals) please enter zeros and submit the form. It helps us to know when you haven’t screened or treated.
